Before you fill out the form here are a few things you should be made aware of below.

I charge a flat day rate, days are broken into full and half,

half day $500 to $700 

Full day $1000 to $1500

a half day is typically 2 - 4 hours

a full day is 6-9 hours

deposits are $250 to $500, they are non refundable and go towards the total price of your tattoo.

Zelle, Cashapp, Cash, Applepay and Affirm are all accepted forms of payment.
